Diabetes, Nutrition and Endocrine Center

Diabetes and other nutrition and endocrine-related disorders can seem like a roadblock to a “normal” life, but Affinity Health System helps you learn to manage your body. We help you regain as much freedom and control over your health as possible.  

Affinity offers medical care and self-management training for diabetes at various locations throughout the Fox Valley.

Team of Specialists

The Affinity Diabetes, Nutrition and Endocrine Center brings together a team of health care specialists including endocrinologists, advanced practice nurses, certified diabetes educators (nurses and dietitians), office nurses and receptionists/schedulers.

Our specialists work closely with your primary care doctor and with doctors who specialize in other areas affected by diabetes, including eyes, feet, kidneys, heart, childbirth and psychology. We also work with Affinity’s Integrative Medicine program.

The diabetes educators at the Affinity Diabetes, Nutrition and Endocrine Center work with pediatric endocrinologists at the Affinity/UW Children’s Health Center in Appleton to care for children with type 1 diabetes.

Tests and Procedures

Affinity uses all types of diabetes monitors and testing devices. We also offer insulin pens, which can help people with insulin-dependent diabetes lead easier, more worry-free lives.

Education

To help our patients with diabetes learn how to best control their illness, we offer an education program accredited by the American Diabetes Association. Each month, we offer classes on cholesterol management, type 2 diabetes, insulin pump training and carbohydrate counting.

The Affinity Diabetes, Nutrition and Endocrine Center also offers community outreach diabetes screenings, as well as presentations on diabetes and other nutrition-related topics.

Self-Management

Successful treatment of diabetes requires the patient’s full-time cooperation. We understand that learning how to take care of yourself after you’re diagnosed can feel overwhelming. We offer training to help you self-manage your illness, including in-depth training about insulin therapy.

Other individual and group diabetes training topics include:

  • blood glucose (sugar) monitoring and treatment
  • benefits of exercise
  • guidelines for healthy eating and dining out
  • meal plan and medication adjustments during illness
  • how to prevent or delay complications
  • diabetes and pregnancy, including gestational diabetes
